Freshslice Pizza Rewards Program: Terms and Conditions

Participating Stores

1. The Freshslice Pizza Rewards Program, (the “Program”) is available through participating Freshslice Pizza locations and is governed by the rules and regulations of the Freshslice Pizza Rewards Program.

The Program is only available to residents of Canada and verification will be sent to valid Canadian area codes only.

Membership Enrollment

2. To participate in the Freshslice Pizza Rewards Program, a customer must become a Member of the Program. Membership is free and no initial purchase is required. Simply download the Freshslice App from the Google Play Store or the Apple App Store, fill out the required information on the registration form, and accept the Freshslice Rewards Terms and Conditions then you will be automatically enrolled in the Program. You can also go to any participating Freshslice Restaurant and pick up a Freshslice Pizza Rewards Card and register your card on line at www.freshslicerewards.com (you must register a Freshslice Reward Card (Hard Copy Card) to redeem Rewards Points).

3. Only one Freshslice Pizza Rewards account is permitted, per customer. Freshslice Rewards Account MUST be verified in order to redeem Reward Points. ANY Freshslice Rewards Card (Hard Copy) MUST be registered and added to a Valid Freshslice Rewards Members Account before any Rewards Card Points can be redeemed.

4. The Freshslice Pizza Rewards Card is not a credit card. The Freshslice Pizza Rewards Card is the property of Freshslice Pizza and it may be revoked at any time by Freshslice Pizza,at its sole discretion.

5. Each Freshslice Pizza Rewards Member (sometimes referred to as a “Member”) will be issued a
Freshslice Pizza Rewards Membership number that has a unique number ascribed to it and is registered in the Member’s name. Freshslice Pizza Rewards Members cannot share their account number and account information with anyone else. Account information, including, but not limited to, the Freshslice Pizza Rewards account password, e-mail address, name, will be protected and kept secure.

6. Freshslice Pizza Rewards Members must advise Freshslice Pizza immediately of any changes of Personal Information (as hereinafter defined) including name, address, telephone numbers and e-mail address by updating your membership account at www.freshslice.com/rewards. If Freshslice Pizza believes that some of its records are inaccurate, it may contact the Freshslice Pizza Rewards Member to verify its records. Freshslice Pizza reserves the right to cancel any Freshslice Pizza Rewards Card for which it has incomplete or inaccurate Personal Information and all Freshslice Pizza Rewards Points redemption on that account may be blocked. The submission of false or fictitious personal information will result in all the Freshslice Pizza Rewards Points accumulated on an account being forfeited. Freshslice Rewards Membership is not permitted if you are an employee of Freshslice Pizza or an employee at any Freshslice Pizza restaurant. Points will not be issued on any negotiated contract orders with Freshslice Pizza or any of its Affiliates and points cannot be collected or used to pay for any taxes.

7. Freshslice Pizza reserves the right to refuse issuance of a Freshslice Pizza Rewards Card to any customer who does not follow the prescribed enrollment procedures.

8. Customers participating in the Program must present their personal valid Freshslice Pizza Rewards Card to collect points and earn rewards. Any unauthorized reproduction of the Freshslice Pizza Rewards Card may lead to legal prosecution and forfeiture of all Freshslice Pizza Rewards Points and membership in the Program.

9. Freshslice Pizza Rewards Membership is only available to individuals.

Collection of Freshslice Pizza Rewards Points

10. Your Freshslice Rewards Account Phone /Email MUST be verified to redeem rewards points.

11. To collect Freshslice Pizza Rewards Points following enrollment, a Freshslice Pizza Rewards Member must make eligible purchases at participating stores using their personal valid Freshslice Pizza Rewards Card or Freshslice Pizza Rewards APP. The Member’s personal Freshslice Pizza Rewards Card or Freshslice Pizza APP must be presented to the cashier at a participating restaurant before the purchase transaction is completed. The original Freshslice Pizza Rewards Card or Freshslice Pizza Rewards APP must be presented at the time of purchase, no facsimile or just the account number will be accepted. In addition, no Freshslice Pizza Rewards Points will be awarded once the transaction is completed. You must Login with your Freshslice Rewards Membership account when ordering On Line to collect Rewards Points for your On Line purchase. You may obtain a code In Freshslice Rewards App, to receive Rewards Points for a Freshslice Phone in Order.

12. Freshslice Pizza Rewards Points will be awarded at the rate of 10 Freshslice Pizza Rewards Points for each dollar spent, on each separate purchase, at participating stores. Freshslice Pizza Rewards Points will be calculated by reference to the net (purchase price less discounts) pre-tax total of each cash register receipt. The total Freshslice Pizza Rewards Points awarded and reflected on the cash register receipt will be rounded down by the next portion of a dollar spent. Freshslice Pizza may offer special promotions from time to time offering additional Freshslice Pizza Rewards Points on certain products (“Freshslice Pizza Bonus Points”). Such Freshslice Reward Bonus Points, once awarded, shall form part of the Member’s Freshslice Reward Card Points.

13 Login with your Freshslice Rewards Membership account when ordering On Line to collect or redeem Rewards Points for your On Line purchase. You may obtain a code In Freshslice Rewards App, to receive Rewards Points for a Freshslice Phone in Order. Where applicable, Freshslice Pizza Rewards Points will be issued for purchases made through delivery service by participating stores, provided that the Freshslice Pizza Rewards Member provides the restaurant with his/her personal Freshslice Pizza Rewards Card and registered telephone number when the delivery order is placed. The Freshslice Pizza Rewards Card or Freshslice Pizza Rewards APP must be presented to the driver for verification, at the time of delivery. You must redeem points in person at any participating Freshslice Pizza Location.

14. All Freshslice Pizza Rewards Points are shown on the cash register receipt for each purchase when a Freshslice Pizza Rewards Card or Freshslice Pizza Rewards App is presented (Scanned) before the completion of the purchase transaction. If Freshslice Pizza Rewards Points do not appear on a receipt due to a technical failure, they will be updated on the cash register receipt at a later purchase transaction date.

15. When a refund is issued to a Member, the Freshslice Pizza Rewards Points will be deducted automatically from the Freshslice Pizza Member’s identification number that was used for the original purchase.

16. All Freshslice Pizza Reward Points totals, as shown on the cash register receipts, will be deemed correct unless otherwise substantiated by the Member, by bringing forward evidence satisfactory to Freshslice Pizza, in its sole discretion. Discrepancies must be submitted to the Freshslice Pizza restaurant where the purchase was made, within 3 days, of the date, on which the discrepancy occurred.

17. Freshslice Pizza Rewards Points are not transferrable.

17. Freshslice Rewards Points are reduced to zero upon closure of a Freshslice Pizza Rewards Membership.

19. Upon the death of a Freshslice Rewards Member, the Member’s account will be closed and any Freshslice Pizza Rewards Points in the account will be forfeited.

20. Should any Freshslice Pizza Rewards Member not acquire Freshslice Pizza Points for 12 consecutive calendar months, the membership will be deemed inactive and all accumulated Freshslice Rewards Points will be forfeited.

Redemption of Points

21. Freshslice Pizza Rewards Points may be redeemed for savings rewards off a purchase at participating restaurant in accordance with the following Freshslice Pizza Rewards Points redemption schedule:

Freshslice Pizza Rewards must be redeemed in person, on line or by phone with valid code, by the Member that has VALIDATED their Freshslice Rewards Account Membership Email / Mobile Phone Number, at any participating Freshslice Pizza Restaurant.

From time to time, special promotions may be run during which one or more redemption level(s) may temporarily be altered and/or suspended, new redemption levels added or points may be redeemed directly towards the purchase of specific products. Promotions may vary from restaurant to restaurant and from format to format. Freshslice Pizza may, from time to time, offer supplementary programs or promotions related to the Freshslice Pizza Rewards Program (“Related Programs”). Such Related Programs may be governed by additional or distinct terms and conditions applicable therein, such terms to be made available at: www.freshslicerewards.com

22. In accordance with Federal and Provincial tax legislation, GST, PST, HST and any other applicable taxes are calculated and payable by the Freshslice Pizza Rewards Member on the full amount of the purchase price before any reduction for redeemed rewards.

23. A Member’s Freshslice Pizza Rewards Points balance will be reduced by the number of points required to obtain the desired point redemption level. Unredeemed Freshslice Pizza Rewards Points plus any Freshslice Pizza Rewards Points earned in the current transaction, will remain in the Member’s Freshslice Pizza Rewards Points account and can be applied to a subsequent transaction.

24. Freshslice Pizza Rewards Points can only be redeemed if the required point redemption level is acquired prior to the redemption transaction. An individual Freshslice Pizza Rewards Member may not make more than five redemptions, at any reward level, within one day. Only one redemption is available per transaction and Freshslice Pizza Rewards Points must be redeemed in person, at any participating Freshslice Pizza location.

25. When redeeming Freshslice Pizza Rewards Points, Freshslice Pizza will automatically select the maximum eligible Freshslice Pizza Rewards Points redemption level available. Alternatively, the Freshslice Pizza Rewards Member may select a different eligible redemption level by informing the cashier prior to redemption of Freshslice Pizza Rewards Points. In either case, the Freshslice Pizza Rewards Member’s account under his/her Freshslice Pizza Rewards Card (identification) number must have the appropriate number of Freshslice Pizza rewards Points. For example, if the Member has accumulated 8,000 Freshslice Pizza Rewards Points, the maximum redemption level available is 8,000 Freshslice Pizza Rewards Points. However, the Member may elect to redeem only 3,000 Points on a $30.00 transaction; in this case, the reduction off the purchase price will be $10.00.

26. When redeeming Freshslice Pizza Rewards Points, the Member may maximize the dollar value of the reward discount. If the Member does not wish to maximize the dollar value of the reward discount, the remaining dollar value associated with the reward discount will be forfeited. For example, if the Member has accumulated 14,000 Freshslice Pizza Rewards Points and wishes to redeem all these points on a transaction with a pre-tax purchase price of eligible products of $50.00, then the reduction off the purchase price will be $50.00 and the difference between the $60.00 maximum reward discount and the $50.00 discount (i.e. $10.00) will be forfeited.

27. Freshslice Pizza Rewards Points can be redeemed in person and on line at any participating Freshslice Pizza Restaurant with proper account verification and login. Freshslice Pizza Rewards Points may not be collected or redeemed by any person other than the Freshslice Pizza Rewards Member. Members may be required to provide proof of identity when redeeming Freshslice Pizza Rewards Points, to protect the integrity of the Member’s Points balance. Each Member is responsible to ensure that all Personal Information is correct and up-to-date and Freshslice Pizza reserves the right to block redemptions where the Freshslice Pizza Rewards Member information is inaccurate or incomplete.

28. In case of a lost “REGISTERED” Freshslice Pizza Rewards Card, upon presentation of proper identification, a new Member’s number will be provided to the Member at any participating Freshslice Pizza restaurant. The accumulated unredeemed Freshslice Pizza Rewards Points balance shall remain intact. If a lost Freshslice Pizza Rewards Card is found and used by any other person, all Freshslice Pizza Rewards Points relating to purchases made by such other person will be deemed used and no credit of points will be issued to the affected Member.

29. Freshslice Pizza Rewards Points have no cash value and are not exchangeable for cash. Freshslice Pizza RewardsPoints cannot be assigned, exchanged, purchased or given by gift. Freshslice Pizza Rewards Points cannot be sold, traded, bartered, rented or otherwise disposed of. Freshslice Pizza Rewards Points are void if sold for cash or any other consideration.

General Information

30. The Freshslice Pizza Rewards Program will terminate upon Freshslice Pizza announcing to Members that the Freshslice Pizza Rewards Program, or any part of it, is terminated, which may be done at any time on 45 days prior written notice (to be provided at participating restaurants, in Freshslice Pizza restaurant flyers, freshslice.com website, Freshslice Pizza Rewards APP and/or through other communications to Freshslice Pizza Rewards Members including via e-mail), (the “Termination Date”).

31. Freshslice Pizza Rewards Members will have a period of 90 days following the Termination Date to redeem Freshslice Pizza Rewards Program points, using Freshslice Pizza Points accumulated up to the Termination Date. Freshslice Pizza Rewards Points will expire at 11:59 p.m. on the 90^th day following the Termination Date and will not be honoured, after such expiration date.

32. If events beyond Freshslice Pizza’s control materially affect Freshslice Pizza’s ability to operate the Freshslice Pizza Rewards Program, the Program may be suspended or terminated without notice.

33. Freshslice Pizza may restrict, suspend or otherwise alter any aspect of the Freshslice Pizza Rewards Program, without notice. Any material changes to these Terms and Conditions will be posted online and a copy of the Terms and Conditions will be made available at each participating restaurant, upon request. 34. The presentation of a Freshslice Pizza Rewards Card or Freshslice Pizza Rewards APP constitutes agreement, by the Freshslice Pizza Rewards Member to all the Terms and Conditions of the Freshslice Pizza Rewards Program.

35. Any abuse by a Member of the Freshslice Pizza Rewards Program’s privileges, any failure by a Member to follow the Program’s rules, regulations, Terms and Conditions, or any misrepresentation by a Member may, at Freshslice Pizza’s sole discretion, subject the Member to expulsion from the Program, including forfeiture of all accumulated Freshslice Pizza Rewards Points.

36. Upon suspension or termination of the Program for any reason whatsoever, Freshslice Pizza will have no liability therefore to any Member and is automatically released from all claims by Freshslice Pizza Rewards Members in respect of such suspension, termination, forfeiture of Freshslice Pizza Rewards Points, and redemption of Points or otherwise in respect of such suspension or termination.

37. Every effort has been made to ensure that the information herein is correct. Freshslice Pizza is not responsible for any errors or omissions.

38. Any waiver by Freshslice Pizza of the strict observance, performance or compliance by a Member with any of the Terms and Conditions contained herein, either expressly granted or by course of conduct, shall be effective only in the specific instance and shall not be deemed to be a waiver of any rights or remedies of Freshslice Pizza as a result of any other failure to observe, perform or comply with the Terms and Conditions. No delay or omission by Freshslice Pizza in exercising any right or remedy hereunder shall operate as waiver thereof or any other right or remedy.

39. By redeeming Freshslice Pizza RewardsPoints for rewards, the Freshslice Pizza Rewards Member releases Freshslice Pizza any and all liability and claims regarding the redemption and use of the reward and any loss or damage caused by goods and services supplied as a reward.

40. Any reference herein to Freshslice Pizza refers to RFSP Equipment & Operating Inc., its affiliates, and the licensed operators of the participating Freshslice Pizza restaurants, as the context requires.
